can this handle 4K gaming?

Yes, this card is a beast. It will also do VR I believe. This is one of the best cards on the market right now. I max out my in game settings and it still runs above 100 frames a second. CPU is ryzen 3600x O.C to 4.4 ghz

Will this fit into a nzxt h1 mini itx case?

the two fan model may fit, but the 3 fan model i would say no way it would fit its a really large card
