Q: What exactly comes in the box? I've seen reviews where this comes with both 125mm pad and 150mm pad. Is this true?

The 6 inch sander only comes with 6 inch pads. It comes with one Abranet pad and 9 sanding discs, all different grits.

Q: Can a 150 run a 125 pad and vise versa?

I assume you mean the gram weight of the pad. There are weights (included with paperwork of the sander) in the form of tiny screws and nuts that you can add to the counterbalance to adjust for the gram weight. The counterbalance is behind the pad. In the paperwork is the screw and nut configuration. It's very easy

No, neither hose or adapter for Festool vacuum. Hope this helps.
