Will this work for a residential heavy door with wooden frame? The door and frame are not flush.

I cant honestly answer that not knowing details of your situation. It is designed for walk-in cooler doors that are about 84" high and about 4' wide. It is designed to pull the door up to the box frame when the door is about a few inches from the frame. It is spring driven, not hydraulic which means it is lighter duty. If your door swings freely with NO resistance then it will probably work.

Does this include hook? Thanks

It did not include the hook - therefore I sent my back for a refund

Is this product in fact a Kason 1095?
