Is this for a 50 gallon tank?

If your water pressure is below 70 PSI, this tank will work. If it is 80 PSI, you will need the larger 4 gallon model. Refer to this guide http://media.wattswater.com/pg-thermexpansion.pdf/.

Do i need this if my boiler has one and water heater is close by

Heating boiler piping system is typically isolated from the water piping system for the rest of the house (such as by a pressure reducing or check valve to drop the house water pressure of ~ 60 psi to the typical less than 20 psi of the heating boiler side). Therefore the heating boiler expansion tank function would not effect the hot water boiler thermal expansion. Heating boiler thermal expansion tanks are typically factory charged to 12 psi while those for hot water tank are charged to 40 psi. ps I am not a professional so you may want to do more research of your own.
