Is there a box or place to enter a date on the top of each page?

No box on the page. The pages are lined so you could put the date on the one.

Will the pen holder hold a pencil?

Yes, I think so, a no. 2 pencil certainly. It's an expandable loop so it will hold a large pen or pencil, perhaps up to 1/4" or so in dia.

Is the cover real leather?

Im not versed on real or fake leather but it definitely feels like quality material and has held up very well over the 4 or 5 months Ive had it.
