Is the 3.5mm plug straight or at a 90 degree angle to cord?

It is straight. However be careful. The ear hooks are adjustable and they slide around when I am working out so they do not stay in place and they fall off.

Is there a microphone?

No microphone but there is one on the two....

Does it fit into the iPhone LifeProof cell phone cases?

For older iPhones that still support 3.5mm headphone jacks, I've found many of the bulkier protection and battery cases require a plug extender for these to work. For newer iPhones, a lightning to headphone jack dongle is required.

How do you keep the earpiece from rattling? I've tightened the screws and it still rattles.
