Can this be bolted permanently on a bench. Is there provision for this in the frame

The simple answer is Yes. The illustration on the Amazon page shows how to clamp to a bench. In the bottom part of the frame you could drill bolt holes and line up with the bench. If you have a workmate screw a 50mm timber square to the frame This will allow you to clamp between the workmate .jaws.

Does this come with the jaw cushions?
